Your approach will be based on the child’s developmental stage:

Review of abdominal surgical scars in adults:

Immediate, full access to the abdomen: trauma, shock states
Open appendectomy
Alternate (more cosmetic) open appendectomy
Nephrectomy
Open cholecystectomy
Pancreatic cancer; Whipple procedure
Liver transplant
Renal transplant
Children’s abdomens are smaller, organs more compact; abdomen more convex
Rare to perform midline laparotomy
More common: smaller incisions in transverse approach give more visualization in children
